Stress hormones and anxiety

Stress hormones play a central role in anxiety. Hormones such as cortisol and adrenaline help the body respond to perceived threat, but when these systems are repeatedly or persistently activated, anxiety symptoms can become more frequent, intense, or difficult to regulate.

How hormones fit the framework: Stress hormones help explain why anxiety can feel intensely physical even when no immediate danger is present. They also help clarify why anxiety may appear to start in the body first for some people and in the mind first for others.

The stress response system

When the brain detects potential threat, it activates coordinated systems designed to increase survival. Two key components are the sympathetic nervous system and the hypothalamic–pituitary–adrenal (HPA) axis. Together, these systems regulate the release of stress hormones.

This response is adaptive in short bursts. It becomes problematic when activation is frequent, prolonged, or triggered by low-risk cues.

Key stress hormones involved in anxiety

Several hormones are commonly discussed in relation to anxiety:

  • Cortisol: released via the HPA axis; mobilizes energy, alters immune signaling, and affects memory and attention.
  • Adrenaline (epinephrine): increases heart rate, blood pressure, and alertness; closely linked to fight-or-flight responses.
  • Noradrenaline (norepinephrine): heightens vigilance, attention, and threat sensitivity.

How stress hormones influence anxiety symptoms

Elevated or fluctuating stress hormones can amplify both physical and cognitive anxiety symptoms. Increased heart rate, muscle tension, shakiness, gastrointestinal changes, and sleep disruption are common physical effects.

At the cognitive level, stress hormones can narrow attention toward threat, increase worry, and reduce cognitive flexibility. This overlap is often discussed in catastrophizing, hypervigilance, and intrusive thoughts.

Body-first and mind-first pathways

In some people, stress hormone shifts appear to precede conscious worry, contributing to body-focused anxiety. Physical sensations may arise quickly and feel unexplained.

In others, sustained cognitive threat prediction activates stress hormone release over time, contributing to mind-focused anxiety. Many experiences involve mixed pathways.

Chronic stress, burnout, and hormone regulation

Prolonged stress exposure can alter how the HPA axis responds to future challenges. This may include heightened reactivity, delayed recovery, or altered cortisol rhythms. These patterns are commonly discussed in stress and burnout.

Cultural and contextual considerations

Environmental demands, social stressors, work conditions, and exposure to instability influence stress hormone activation over time. Cultural norms around productivity, rest, and emotional expression can shape both stress exposure and recovery patterns.
